Hazard Identification and Risk Assessment SOP
Purpose: To identify and assess potential hazards in the workplace to prevent accidents and injuries.
Scope: This SOP covers all areas and activities within the organization. The Occupational Health and Safety Officer is responsible for conducting regular inspections, identifying hazards, and assessing risks.

Hazard Identification Process
- Planning:
- Schedule regular inspections of all work areas and activities.
- Develop a checklist based on known hazards specific to the industry and workplace.
- Inspection:
- Conduct walk-through inspections of the workplace.
- Observe work practices, equipment, and environmental conditions.
- Engage with employees to identify any hazards they have noticed.
- Documentation:
- Record identified hazards using a standard hazard identification form.
- Include details such as location, description of the hazard, and potential risks.
Risk Assessment Process
- Risk Evaluation:
- Evaluate the likelihood of each identified hazard causing harm.
- Assess the severity of potential harm or injury.
- Use a risk matrix to categorize risks (e.g., low, medium, high).
- Prioritization:
- Prioritize hazards based on their risk rating.
- Focus on addressing high-risk hazards first.
- Control Measures:
- Identify existing control measures and evaluate their effectiveness.
- Develop additional control measures if necessary, using the hierarchy of controls:
- Elimination
- Substitution
- Engineering controls
- Administrative controls
- Personal protective equipment (PPE)
Implementation of Control Measures
- Action Plan:
- Develop an action plan for implementing control measures.
- Assign responsibilities and timelines for each action item.
- Communication:
- Inform employees about identified hazards and the measures being taken to control them.
- Provide training on new procedures or equipment if necessary.
- Monitoring:
- Regularly review and monitor the effectiveness of control measures.
- Adjust or improve control measures as needed based on feedback and observations.
Review and Update
- Periodic Review:
- Conduct periodic reviews of hazard identification and risk assessment processes.
- Update risk assessments to reflect changes in the workplace, such as new equipment or processes.
